The following pages contain some pre-screening questions to determine whether you may be eligible to take part in the AMPLITUDE clinical research study. If you would like to participate in the online pre-screening, you will need to provide certain personal information such as your answers to some health-related questions (to determine eligibility) as well as your name and contact information (if you are potentially eligible and choose to be connected to a research site).

Vertex Pharmaceuticals Incorporated (“Vertex”), the sponsor of this study, is the data controller in respect of the processing of your personal information. Vertex has engaged WE ARE COUCH LTD (“COUCH Health”, “we” or “us”) to assist with clinical trial recruitment for the AMPLITUDE study by collecting pre-screening information for the study through this website.

The pre-screener information you submit online will not be shared with Vertex in any way that could identify you. COUCH Health, which is located in the United Kingdom, will store this information, as the operator of the website, and will only share information in aggregate with Vertex. If you are outside the United Kingdom, please note that the European Commission has found that the privacy laws in the United Kingdom adequately protect your information.

We will use this information with your consent to evaluate your eligibility for the AMPLITUDE study. In the event that you are found potentially eligible based on your answers to the pre-screening questions, you will have the option to provide your contact information (such as your name, email address and/or phone number, and post code). We will use and share this information with a research site to allow them to contact you for further assessment of your eligibility. In the event that it appears that you are not eligible for this study based on your answers to the pre-screening questions, you will not have the option to be connected with a research site and we will delete or anonymise the information you provided in the pre-screening questionnaire.

In either case, you can elect to have your personal information (including your answers to the pre-screening questions) deleted. If you do not elect to delete the information, we will store it for as long as necessary to fulfill the purposes listed above, in accordance with applicable laws.

You may withdraw your consent at any time and upon withdrawal of your consent we will promptly delete your personal information except to the extent required to comply with our legal obligations.
